Ticking on Cold Start: Exhaust Leak or Something Worse?

A tick that is loud cold and fades as the engine warms is usually an exhaust manifold leak. How to tell it from a lifter, injector or rod, and why fix it.

A ticking engine on a cold morning gets people worried about the bottom end. Most of the time it is an exhaust leak at the manifold, and the giveaway is that it gets quieter as the engine warms up. Here is how we tell the noises apart and why an exhaust leak is worth fixing even though the car drives fine.

Why exhaust leaks tick when cold

Exhaust manifolds, particularly cast iron ones, crack over years of heating and cooling. Manifold studs also break — very common on some trucks and SUVs. When the engine is cold, the crack or the gap is open and pulses of exhaust puff through it with every cylinder firing, which sounds like a tick or a chuff timed to engine speed. As the metal heats and expands, the gap closes and the noise fades or disappears. That pattern is almost unique to an exhaust leak.

The noises it gets confused with

Hydraulic lifter tick

Also loudest on cold start, but it comes from the top of the engine under the valve cover and it usually quiets within seconds to a minute as oil pressure builds — not gradually over several minutes as the metal warms. Low oil level or the wrong oil makes it worse.

Injector tick

A steady, even, metallic tick from the fuel rail at all temperatures. It is normal on many direct-injection engines and sounds alarming to anyone new to them.

Rod knock

Deeper, heavier, from low in the engine, and it gets louder under load and with engine speed rather than quieter with warm-up. This is the one that matters. If a knock gets worse as the engine warms and you can feel it, stop driving and call.

Piston slap

A hollow, rattly sound on cold start on certain engines that fades when warm — similar in pattern to an exhaust leak, but from inside the block. Usually harmless on the engines known for it.

Why fix an exhaust leak

  • Exhaust fumes in the cabin. A manifold leak is at the front of the car, right under the fresh-air intake for the ventilation system. Carbon monoxide is not something to live with.
  • Check engine light. A leak ahead of the oxygen sensor lets air into the exhaust stream, which reads as lean and can set fuel trim codes and eventually a catalytic converter efficiency code that was never the converter's fault.
  • Emissions inspection. Mecklenburg County is an emissions county. A light caused by a leak is still a fail.
  • It gets worse. Cracks grow. A broken stud lets the manifold warp, which turns a stud repair into a manifold replacement.

What the repair looks like

For a leaking gasket or a broken stud with good access, it is a driveway repair. For a cracked manifold the manifold is replaced, which is on-site work on many vehicles and shop work on the ones where the manifold is buried. Extracting broken studs from a cylinder head is skilled work, and we will tell you how many are broken and what that means before starting. Leaks further back — at a flex pipe or a joint — are usually quick.

Frequently asked questions

Is it safe to drive with an exhaust leak?

For a short time with the windows cracked, yes. Over a Charlotte summer with the A/C on recirculate and the windows up, you are breathing whatever gets in. Schedule it.

Will it fail the NC inspection?

The safety inspection checks the exhaust system, and the emissions inspection checks the check engine light. A leak can fail either. See our post on passing the NC inspection.

Can you tell me which noise it is over the phone?

Sometimes we can narrow it from your description. We would rather put a stethoscope on it, which takes a few minutes at your driveway anywhere in Charlotte or Mooresville.
